FAMILY & COMMUNITY HEALTH
Health Happens Together
Healthy communities are built through more than individual choices.
The Destiny Program provides health education and family programming designed to make wellness practical, accessible, and connected to everyday life.
Intertribal Fitness
Intertribal Fitness provides accessible opportunities for youth, adults, and families to build strength, improve physical activity, and develop healthy habits in a supportive community environment. Programming is designed to meet participants at different fitness levels while encouraging consistency, confidence, and long-term engagement in physical activity.
Family Healthy Weight Program
Our family-based healthy lifestyle programming supports children and caregivers in building healthy habits together through physical activity, hands-on nutrition education, and family participation.
Diabetes Prevention
We support lifestyle-change education designed to help people at risk for type 2 diabetes build sustainable habits related to nutrition, physical activity, weight management, stress, and overall health.
Blood Pressure and Brain Health Education
Programs provide education that connects brain to heart health education and support around blood pressure management, nutrition, physical activity, self-monitoring, and other heart to brain-health behaviors.
Nutrition, Cooking and Community Wellness
Food is connected to health, family, culture, community, and place.
Our food and nutrition activities provide hands-on opportunities to build practical cooking and food skills, strengthen nutrition knowledge, increase access to healthy foods, and reconnect food with health, family, culture, and the broader food system.
Through cooking, food demonstrations, nutrition education, and culturally relevant activities, participants gain skills they can use in everyday life while strengthening connections to food and community.
Heart Health & Community Preparedness
Cardiovascular health is an important part of our prevention work.
Through collaboration with the American Heart Association and local partners, The Destiny Program supports blood pressure awareness and education, community health outreach, cardiovascular disease prevention, and efforts to strengthen cardiac emergency preparedness.
This work connects individual prevention with broader community capacity by helping increase health knowledge, local resources, and readiness to respond during cardiac emergencies.
